 At Reaching Hearts, our mission is to empower every individual with the support, resources, and opportunities they need to live as independently as possible and on their own terms. We are committed to fostering dignity, choice, and personal growth, helping each person achieve their goals and thrive within their community. 

Our Services

We offer multiple services that fall under  Texas Home and Community-based Services (HCS) and Texas Home Living (TxHmL) Medicaid waiver programs.
